Each completed document carries a signing record: who signed, when, the network address and device used, and a fingerprint of the original and signed files. A completed document can't be edited; change the text and re-issue instead.

Opening a document isn't the same as acknowledging it; when staff must confirm they've read a policy, use a policy acknowledgement in Compliance.
